[graphite][vulkan]: handle explicit YUV VkFormat resolves

Some drivers may resolve external hwb formats beyond the required
AHardwareBuffer Format Equivalence table to an equivalent VkFormat
instead of VK_FORMAT_UNDEFINED.

For YUV VkFormats, provide the needed YCbCr conversion info.
